Some countries missing on checkout #185

We noticed this specifically for United Arab Emirates, but there are others
as well.

What steps will reproduce the problem?
1. Purchase an item and checkout.
2. Select the drop down box for country.

What is the expected output? What do you see instead?
United Arab Emirates should be here.  It is in the countries list.  But it
is not listed

This also applies to countries that are not long names...apparently our country 
list
is out of date also.

So...looking at this list...not sure that all of these qualify as countries
(Scotland, Wales) and I have no idea what Yap is. 

In any case, the country list is out of date. Perhaps:

or

in any case...we need an update.

I have fixed at least one of the causes of this, the countries with names 
larger than
18 chars and no commas on them where not being included.
Some of the countries in this list are actually not in the iso standard.
I still need to check if there are conuntries in the standard that are not being
included.
