Separate functionality in /add-providers page #22

Currently the different functions (Add, Bulk Add, etc.) are all accessible from the page without any seperation between them.

This makes it harder for a user to understand where one form begins and another ends.

As a workaround until we change the design, let's wrap each function in some box.
