There is a setting in the itw-settings application to configure this behavior.
It is in the tab Desktop Integration
There's nothing called "itw-settings" in the Start menu. I'm guessing you meant "OpenWebStart Settings" because it popped up unexpectedly:
I changed it to "Ask user" but I still get prompted every time I start the program.
With OWS on master (to be released soon) if the shortcut and desktop icon have been created once, OWS does not ask for them to be created, it just updates them.
However if the user chooses in the dialog not to create DesktopIcon then this decision is NOT remembered and OWS will ask again the next time the app is started.
Reproduced here. It will only start asking you to create shortcuts, when you give in and let the software create shortcuts. If you uncheck any of the shortcut checkboxes, it will ask you again next time.
We will fix this issue in a future release.
The problem is still present in version 1.4.0

It's not really important, it's just concerning and confusing.
OWS 1.10.0 - problem still exists

Software Environment:
Expected Result: OpenWebStart should only ask for my shortcut preferences one time.
Actual Result: OpenWebStart asks about shortcuts every time I start the program.
