Update upload data in map control panel #283

Problem

From the feedback 

I tried to Upload data using the side button  -i.e. one site only. 
I completed the fields, but could not find how to submit? So had to exit.  
Please check this.  Also in the future there should be some predictive text and drop down lists, and the origin should be defaulted as this won't change, it can actually be removed I guess.   
Also we need a tooltip to say that Upload data on left is for a single site, while upload data on top menu bar is for multiple records....I couldn't test this though as I had the login issue as explained above

Size of upload form reduced. Tooltip changed to "add single occurrence"
