Rename Shell → Application #422

The word 'shell' was mis-used for multiple contexts, thus:

struct Shell → Application
trait ShellSharedErased → AppShared
trait GraphicalShell → AppGraphicsBuilder
mod kas::shell → app
In documentation, usages of 'shell' are roughly split between 'application' and 'graphics backend'.
