[ISSUE-18] Prepare decision log #137

Resolves #18

Scope of changes:

I added:

new folder for documentation
template for decision log (to be copied to decision log in each edit)
first decision

How to use it:

Copy template from the templates folder and paste to the DECISION_LOG.md
