Allow clients to enter their contact information themselves. #1

This includes three changes:

1) Make the client page more user friendly

2) Make the SCA name something more general

3) Add a "client" role that only allows access to the client pages.

1 and 3 are done. For #2, the client page does not mention the SCA Name, it is entered when forms are added.
