Create a default issue template

Description

It could be beneficial to create a default issue template. This would create a consistent format for all issues created.

Steps to Reproduce

• N/A

Acceptance Criteria

• A template has been made to auto fill new issues with several headers and a description of what should be beneath each.
• there should be documentation changes in the wiki corresponding to the new template

Solution / Pointers

• should be able to be done the same as #98
• The format should look something like this issue (meta)
