Refine logs and log files #107

I adjusted the log to find out the cause of problems such as https://github.com/kc9wwh/macOSUpgrade/issues/44 or https://github.com/kc9wwh/macOSUpgrade/issues/105.

Specifically I tried to write startosinstall standard error output to the log file.
