This issue appears to be a Survey123 bug, specific to iPad. Working on issue report for Esri.
@KCI-Ablowers I made some changes to your form. Please test again with the iPad. In the Description of test field, enter the model and nature of what you're testing.

works fine on iPhone X as long as JPG is used
This appears to be resolved by @KCI-Ablowers by deleting and reinstalling the app on the affected iPad.
I reviewed some of the back end of the Survey123 structure and function with @dobrochtaKCI.
The surveys I submitted appear in the 'NPDES BMP Inspection 20180308 Maryland SHA' Inspection attribute table (Seven total). All of the data appears to be correct except for the photos. When viewing the related tables that should have photos, they all appear blank. Example below-

I am not sure if this is the only culprit, but it could be the cause of the "unable to complete operation error and it appears to "crash" when sending as soon as it attempts "Attachment X our of X"-
