Interesting. This looks like a Survey123 app problem. There is no setting that controls this, that I'm aware of. I'll ask Esri.
if there is no way to correct, i assume that the photo is stored locally somewhere since it was taken with the device. can the user easily find the photo on the device and add again. The scenario I see is that the rating is changed to a lower value in the vehicle or in the office (not at the site), and the photo is lost.

change rating for question that has a photo to a lower rating that does not require a photo
resubmit the survey
load feature service in AGOL and view that question's related table
the photo remains in the related table, but it lost in Survey123. Photo can be deleted from the related table in AGOL.
@KCI-Ablowers To your recent comment, that issue is documented in #49 and #33. This issue thread is regarding toggling to a no-photo-required rating within an active form.
If I rate something as a 3 and add a photo, then change the rating to a 2, then back to a 3, 4, or 5, the previously uploaded photo is removed. Shouldn’t the photo stay unless the user deletes it? If a user accidentally changed the rating to a 2 then back to a 3, they are forced to reload the photo. The photo should remain unless the user deletes it.
