I'm not sure; I've never tried from terminal vim. It should work in principle but for the reverse search I think MacVim.app is hard coded as the app to search

Seems that this is more a macvim problem — when using CLI macvim, it doesn’t start a server. Only in app macvim, it comes with a server name VIM.
