Covers aren't included #1

For sites that support it, a cover image should be downloaded and included in the epub.

If they don't include it, some simple cover-page should be generated. (title \n by author in big letters in the middle of the page? Probably acceptable.)

As of bff375a8e732a76ca71f6e609c3b8571b7773f6c covers are being generated. Still need to obtain covers for sites that have them.
