The content of the user report card should change to the following.

 - Last Appointment date
 - Last Re-exam date
 - Click to see the Progress chart (this will be a new feature)
 - Click to write an email to your doctor.

These should show using the graph on the top.
 - Total Number of session so far
 - How many weeks into the treatment
 - How many weeks left

Alpana can you draw this and give us an outline of how this should look
like. also, once you have a template pls work with Neel before you assign
it to Dev team.
