Problem with Payments.

Treatment plan length is calculated as the following.
It's the maxima of the number of session (not weeks) for any service. So if the 
Acupuncture is the maximum occurring service then count the number of 
total acupuncture session and that's our Treatment Plan Length. 

The re-exam service should be added differently. It should say 

Total number of re-exam during the duration of treatment _______________ 
and it should default to Treatment plan length/8.

The Print Function should require name as well just like how save is.
