Problem with New Patient UI #42

First think is the size need to be made smaller, it's crossing over the 1024 
limit just by a little bit.

One of the places the long is typed as Loang, please correct that typo.

In the section what it ask for GLasses, there should be a text box to enter at 
what age. YOu have the label but the txtbox is missing.

Hemoroid is spelled wrong.

The buttons need to become bigger in each page.
