On treatment page, some UI changes.

Herbal Treatment does not default to 3, it should.

The label FOrmula should be changed to Treatment Priciple at all places. I 
think there are a few places where this need to be corrects. Also on herbs 
pages.

The Other tab and it's description are out of alignment.

the Powder checkbox also need to have a choice in drop down form, the 
choices are 
1. 0.25 bottle
2. 0.5 bottle
3. 0.75 bottle
4. 1 bottle
5. 2 bottle

The price for half bottle is $12.

Correction: The price for 1 bottle in $36
And default should be 0.5 bottle.
