Data loss in NPI

Second page of NPI - # glass per day: looses data when navigated to next page.

steps to create -
1. fill some number in # glass per day optin - I provided 4
2. Go to next page
3. Come back to the previous page
4. Data is no more there.

Also, I chceked the Other (specify) checkbox under "your past medical
history" and typed come value in the text box. Now after navigating back to
this page, the checkbox shows unchecked.

Health Insurance Info on second page of NPI is also getting lost -
1. Insurance Co. Name
2. Policy #
3.Address
City, State, ZIp
4. Phone 

On page 4 of NPI, all the data get lost by clicking prev and coming back to
this page again.

I still see data loss with prev and next clicks-
1. Other (specify) checkbox on second page of NPI looses the check.
2. # glass per day on 2nd page of NPI- looses data

Works fine in Test server. I didn't see any data loss in NPI
