Report Card should be populated with relevant information. #91

Currently Report Card do not show any information. 
FYI - "Missed appointment notification" needs to be discussed over for what 
and how to show this info. This will be implemented as per the decision 
taken.

I looked at the design docs and basically the information that is present on 
the UI
is correct and as per the design doc. All we need to do it to populate it with
relevant information/data.
Currently Report card shows these info -
1. When is the next examination due
2.How much is left on your account
3. How many sessions have you used
4. Missed appointment notification

Each of these should have a line and then the correct data after the line.

I saw some changes/data on UI but not sure if anyone has verified the fix or if 
the fix 
is complete.
