User module > Prescription page - Formula, Herbs, Portion title should be on top of the table

Currently whenever we fill the table with some info, Formula, Herbs, Portion 
title goes to the button row. It should always be shown on top of the table.

Steps to re-create -
1. go to http://body-fs.appspot.com/pages/user/prescription.zul
2. login as alpana2.wadhwa2/alpana2123
3. Go to Weekly Visit > Prescription page
4. The Formula, Herbs, Portion title is moved to the bottom most row of the 
table.
These should be on the top row.
