Update all the stuff #95

switch to ESM, require Node.js 12
switch to GitHub Actions
add coverage with c8
update all packages to the latest version

Notes:

I couldn't make the Windows CI to pass even though the runners should have symlink support enabled
I couldn't remove make-dir and pify packages; would be nice if we could reduce the deps later
I doubt this package is maintained, but I don't have the time to maintain a fork myself. If anyone wants, feel free to cherry-pick my patch
