rm2-cal - first page not appearing in TOC #1

The TOC entry for the first page in a generated PDF isn't showing up in Preview.app from macOS 14.1.

I don't remember noticing this before, not sure if this is a problem with how I'm building the TOC or a bug in Preview.app.
