Documentation should contain a full description of markdown syntax, noting 
pandoc extensions, and being explicit in places where Gruber is not.

Done as of 3a3b4849578a8699265b4bba00927ffade14a218.
The README now contains a full description of pandoc's markdown (including the 
non-pandoc-specific parts).
Also, a man page pandoc_markdown(5) is created, so you can 'man 
pandoc_markdown'.
