CALS tables in docbook writer #294

Docbook initial format for tables is CALS (http://
www.sagehill.net/docbookxsl/Tables.html#CALSvsHTMLtables) and docbook
lately added xhtml tables support.

As a result, most of the tools working with docbook only understand
CALS syntax (and, as a pity, XHTML to docbook seems to be a never
solved exercise).

I know that the fact that other tools does not support xhtml tables is a bug in 
these tools, but I think this could be an enhancement to allow a switch in 
pandoc to support this syntax.
