Login/Signup page for user

Login and signup page to enroll themselves and to get details 1.Name 2.Address,(State and District) 3.Adhar card number or any valid document (To verify whether User is original user and to ensure the complaint is genuine) 4.Phone Number 5.Issue 6.Date of the issue

This will make the process simple like He/she Need not to enter the details everytime when they have the issue and want to report it When they have account they just need to enter the issue (Problem).

To create Login/signup page we need to implement it by front-end(html,css) and backend (php,mysql)
