What about rod of recall? If one player is out of lives, the other should still be able to escape, right? If the player wearing the Cloak of Deception dies, can the other take it off to stop being treated as undead (such as for getting a map aura heal)?
What if everyone shared the same inventory, using their combined strength for the encumberence limit? It would better match with the single quest list, be easier to manage plot items (although only one player could get an effect from the sandworm heart?)
Even useful and sane ones like transmogrification chest, scrying orb
even specific plot items?
yes, transmog/scrying orb should be linked to by both players or passed every turn.
