匯入 國際疾病分類 #28

資料在 http://www.nhi.gov.tw/webdata/webdata.aspx?menu=17&menu_id=1042&WD_ID=1042&webdata_id=3987

一樣是跟著健康存摺資訊找到的，但不確定實用性如何，如果使用頻率不高也許就以檔案形式存在，需要的時候直接找檔案會單純些。

ICD10 還沒上線, 現在用的是ICD9

看起來還是處理 ICD9 會單純些
