Configure server to use Keycloak

[ ] Define oidc-servers.json configuration file for all deployment environments
[ ] Configure SMART inbound auth module in properties file
[ ] Configure FHIR endpoint to use SMART inbound auth module in properties file
[ ] Implement auth.js with post auth callback. Must add appropriate FHIR role
[ ] Add Keycloak to docker-compose.yml for local development
[ ] Update development section in README
