Develop and test Kids First episode of care conformance resources

As part of longitudinal data modeling in FHIR, develop/test Kids First episode of care conformance resources. Consider the following steps:

• [ ] Consult with the followings about the discussed specification
  • FHIR EpisodeOfCare
• [ ] (Optional) Create a new kfdrc-episode-of-care profile
• [ ] (Optional) Create any necessary Extensions for added attributes
• [ ] (Optional) Create SearchParameters for the above Extensions
• [ ] (Optional) Create any necessary CodeSystems and/or ValueSets
• [ ] (Optional) Bind the above ValueSets to the new Extensions
• [ ] Create example resources
• [ ] Validate the above resources with the FHIR IG publisher
• [ ] Test ingest and search
