Remove build and push of phenopackets/kidsfirst images #89

We don't need to build and push separate images for each of these servers anymore. Now we can build one image and pass in runtime environment variables to point the dashboard to the FHIR server. This was implemented in #82

We still need to remove build and push of the Kids First demo server running in dev on the single EC2.
