Inspired by #116, this updates some of the readme structure and simplifies parts of it to make it easier to read and use. I didn't actually create a CHANGELOG file once I realized that the releases contain the change notes. If we were to have both we would have to maintain both which would be more difficult than it is probably worth. Open to thoughts about that though.
I figured I would use this as a first pass to do some basic cleanup and then we can take a look at the possibility of doing more re-arranging of sections in the readme.
