Refactor mesh to model/mesh arch #97

Description:
 Now mesh will contains a single group of buffers.
 And for vertex sharing and multiple material support, it need to be refactored to a model-mesh two level arch.
 A simple solution as following:

 model: buffers, texture buffers, materials, meshes
 mesh: material pointer, texture pointer, buffer description, input layout, material pointer.

 And mesh is the unit of drawing.
