Crash detection #61

Adds detection for crash with 4 modes: -mode shown - adds an indicator to score and plays the topout sound effect on a detected crash -mode topout - ends the game on crash detection -mode crash - crashes the console on crash detection -mode off - does not indicate crashes

Some TODOs:

~~re-enable pressing select to turn off the nextbox outside of qual mode~~
~~adjust indentation so formatting is uniform~~
document feature in CHANGELOG and README
~~benchmark cycle performance change and check PRG usage~~

this is a personal note, I don't mind doing these
