Sync gh-pages and master #16

Currently the CNAME file is only in gh-pages because I thought it'd be neater (seeing as it's only relevant to GitHub hosting) but it's probably better for consistency and it means I can fast forward gh-pages from master for minor changes that don't need a pull request's worth of documentation.
