Reflected the suggestion at https://github.com/klabhub/neurostim/pull/182. Fixed the bug when using the 3rd input to digitalOut

As I am not usng MCC , I cannot judge whether these changes are safe. @cnuahs, can you check?

I'm not using MCC either. Actually, I don't have easy access to an MCC device at the moment to even test this.

@dshimaoka Any updates on the status of this PR? Seems none of us have means to check, so it comes down to whatever you can do at your end I think.

This PR was originally made according to suggestion by nic and tested by me in the acute lab mcc. @nicholasprice, how about checking this PR and amending it if necessary? It is now left opened in the acute lab.
