Comments from review:

Replace INLINEABLE to INLINE, as discussed (if I understood correctly).
Add atWithOrElse that used callbacks inside.
Split insert and delete into multiple pieces.
alter and unionWith functions were refactored accordingly.
Signatures added for rest of tests.

Function arguments put on different lines (it is better for haddock phase).
insert and delete restored to previous state (only PRAGMA changed).
alterM function added.
