Create a form to add new sources

The form should contain the following input elements (bold is required):

• Unique identifier for in Dexter (auto generated and read-only)
• Persistent identifier to the source
• Title
• Description (can also be used as a general notes field)
• Creator (the creator of the source)
• Subject (keywords, can also be used to filter the sources)
• Rights
• Access (this works differently compared to the collection level. You can have a closed collection, but the sources can still be open access)
• Created (when is the source made)
• Spatial (the spatial space of the source. E.g. Tunisia, Turkey, Algeria, etc.)
• Temporal (temporal range of source. E.g. 1920–1960)
• Language (all languages of the source)
• (Contributor) (optional for now)

Behind the scenes, the following elements need to be added to the metadata:

• Persistent identifier to the source in Dexter
• Last updated
• Exact creation date and time of source
• Which user created the source
