Improve readme #179

This should make the README more relevant to people who are coming to the repo. Also fixes a few small issues with the other markdown files.

I think it works in both locations - and at any rate, now that the integrated version is a thing I don't really expect many people to come to the repo anymore, let alone the PyPI package...
