In group LessWrong, folder Approved says it contains 54 stories, but only 23 are visible. (This isn't due to the View Mature filter; I have View Mature checked.)
This story doesn't show up within the Approved folder, but if I attempt to add it to Approved, I get a message saying the story is already in that folder:
