Social Accounts provider_name,provider_id pair is not unique in production database #159

Need to investigate whether this is actually a problem, and potentially fix.

After this is done, introduce a unique index on the pair.

This is no longer desireable, due to our setup with all the knoda accounts sharing twitter. closing.
