Add HGVS columns #73

Fix #71

Currently using beta versions of react-mutation-mapper and cbioportal-utils. We can upgrade to the release versions once we merge cBioPortal/cbioportal-frontend#3500

This PR also includes some minor gene page improvements listed in #66:

Rename % to % Prevalence
Hide dbSNP ID and ClinVar ID columns.
Only show Pathogenic Germline by default
