Fixing reference paths

Summary

Fixing remaining reference paths since the previous PR (#124) didn't fix reference paths with anchors.

For example the previous PR was doing this:

With anchor before: xref:manage-apps.adoc#_upload_an_app[multiple versions] after: xref:manage-apps.adoc#_upload_an_app[multiple versions]

Without before: xref:manage-apps.adocp[multiple versions] after: xref:apps:manage-apps.adoc[multiple versions]
