refactor the different Close button behavior #1110

After implementing #972, we should now refactor the Close button mess. We have 4 different close button behaviors now and it's hard to understand which is for which view. (Not much work).

use css classes: close-form-complete, close-form-participant, close-form-regular, close-form-read

[x] set classes on server
[x] change css
[x] change event handlers and remove client-side dynamic Close-button-augmentation code
