@pbowen-oc, what do you think of this design (used in Survey123):

What I like about it is the additional info (max file size) which is more than we could fit in a button. I think the default enketo strings would not include file formats, but that something you could add in the translation file.

[ ] fix issue with max size being asynchronously determined (so widget shows default 5 MB value)
