Update barcode question type label to mention QR code #1142

@jnm @tinok any idea what is this about? This is how barcode looks right now:

@magicznyleszek Some people consider barcodes to only mean the 1D supermarket variety, and don't think that more practical and sexier 2D barcodes (e.g. QR Barcodes) are implied by this label.

Some people therefore don't realize that you can use QR codes with this question type.

The idea is to either change the label to Barcode / QR code and/or update the icon to look like a QR code.
