Working also with other markdown file extensions #54

Hi, I found Makodo wonderfull but i have the problem that all my markdown files were created with the .mkd extension, and Makojo only open/saves files using the .mdk extension. That is a little cumbersome because i need now to search for files in my computer using both extensions.

Is it possible to incluse other typical markdown extensions, as .mkd and .md?

For anyone search for the support of .md, makodo supports to include markdown files. I found it quite convenient to use an index.mkd as TOC and include .md as one might do in writing a latex book.
