the queries table needs it's primary key altered. #59

The queries subsystem is crap. The tab doesn't work right and the table
won't let you save two queries with the same name even though the clashing
name is hidden because it's been de-activated.

At a minimum, fix the primary key so that it's just qid, not (qid, name)
