Process MathML elements #19

This fixes #11:

copies presentation MathML from nxml files
removes the namespace
add the new custom type pmml to the Mediawiki Math tag to give the MediaWiki:Math extension a chance to treat the input as mathml input rather than tex(vc) input

Yes I checked that. It works good for me. < > was no problem

Hi, @physikerwelt. Looking at this again, it occurred to me that this kind of serialization is something that we already do for HTML tables, and we have a module that we use for that: https://github.com/konrad/JATS-to-Mediawiki/blob/master/lib/serialize.xsl. I think we should reuse that template, since that is what it is for.

Also, there's some active development going on over at Klortho/JATS-to-Mediawiki. I'll write a ticket over there, and point here.
