GitHub released some fancy new Issue Forms that make it easier for people to report issues, let you set up required fields, and give in-line example placeholders (that the user can't submit the form with).
I did my best to stay true to the current issue template, I directly copied things where it made sense but I also included a few extra things to try and keep everything consistent between reports (i.e. suggesting that people should use /version-style commands to avoid getting differently formatted answers and not enough detail).
I completed an example form (where I didn't actually follow the form) here so you can see what it looks like filled out. You can also of course create an issue to see how it looks from that side of things.
