This is deprecated -- hopefully not getting used in production by anyone anymore.
However https://www.npmjs.com/package/koop-server indicates it's still getting downloaded (500 downloads in the last month).
I think we should move this to koopjs and have a plan for retiring it entirely (deprecation warning on npm and when installing, eventual deletion).
